Following are some Administration Issues that are up to you as an employee to take care of.

 

 

 

CHANGE OF ADDRESS/CONTACT NUMBERS/EMAIL ADDRESS:

    •  Notify the office AND change the information on the FL1-website

CHANGE OF BANKING INFORMATION

    • Notify the office to fill out the proper paperwork. 

PAYROLL INFORMATION

    • It is the responsibility of each team member to secure their own access to the federal payroll link online.  It is also each members responsibility to check your pay period statements.  The official link to the Defense Finance and Accounting Service (DFAS) where the Department of Health and Human Services employees are paid is www.mypay.dfas.mil    Your password (if you set it up) should be your six digit birthdate plus fl1. If you have not set this up yet, please contact the office.

TERMINATION FROM THE TEAM

    • If you would like to resign from the team please send an email to the Administrative Officer ( ).  The email must have the term "resign" in it.  We hate to see you leave but we know circumstances change.
